2009-2010 NEW STUDENT REGISTRATION

THE FOLLOWING INFORMATION IS NEEDED TO REGISTER:  

1. Birth Certificate
2. Social Security Card
3. Up-to-date State of Alabama Immunization Card (blue)
4. 2 Proofs of Residency (Lease or Deed and Utility Bill)
5. Transcript and last report card.
6. Guardianship or Custody Papers, if applicable.
7. Driver's license or official photo identification of the person enrolling the student
